Type locality: "2.3 km northeast of Blue River Reservoir, Blue River Ranger District, Willamette National Forest, Oregon" [USA]; (Sheath, Whittick & Cole 1994: 1) Holotype: Fig. 2: R.G. Sheath OR 114; 31 March 1992; UBC; A80770 (Sheath, Whittick & Cole 1994: 2) Notes: Originally collected by Alan Steinman in 1986 and then recollected by R.G. Sheath on 31 March 1992. The latter collection was designated the holotype.
